chore(github): convert issue templates to YAML forms and disable blank issues

Replace markdown issue templates with GitHub YAML form templates for structured bug reports and feature requests. Add config.yml to disable blank issues and enforce template usage. Co-Authored-By: Claude Opus 4.6 (1M context) <noreply@anthropic.com>
