Miguel Angel Simon Sierra 8ed15a6b90 fix(studio-server): a Studio element edit never overwrites a write that lands while it is applied ...

applyFileMutations now refuses any file that changed since the caller read its before (a FileChangedError), not only when the caller passed a version. The element-patch routes re-read and re-fold their patches on that conflict, so both the edit and the other write are kept, and answer 409 after three raced attempts.
