* fix(cli): media-treatment reports its action from every target and documents the per-element merge

* fix(cli): media-treatment patches each match by its stable id so one write cannot redirect another

* fix(cli): media-treatment sets every match in one parse instead of re-finding elements

* fix(cli): media-treatment refuses media that is not an HTML element, as before

* chore(skills): refresh the media-use manifest hash for the treatment reference edit
