fix(skills): no plan step and no money talk; credits come up only when a step is refused

The plan is picked inside the sign-in, so Reverse starts once signed in. No prices,
estimates or credits-used report before, during or after a run.

Use local tools only when the person asks, not as a fallback.
**Host apps.** Inside an app with its own media tools, use those first. When `HEYGEN_API_BASE` is set, the host pays for HeyGen: don't ask for a sign-in, and when a call is refused, relay the host's message as written and stop.
**Generate.** `npx hyperframes media-use generate` makes images, shots, talking photos, voices and music (`references/heygen.md`).
**Credits.** Plan for the best result; don't swap in cheaper or free options, or drop items, to save credits unless the person asks to save cost. Don't quote prices or ask for a budget first; at the end say what the run used and what is left (`auth status`). A call refused for credits names a `billing` command: tell the person why, ask them to run it the same way, then retry the step. `--provider <name>` pins a generator the person names. `--local-only` skips every network provider.

A plan, Free included, comes before any work. Paying is the person's step, like signing in: ask them to run the
command (an agent's own purchase is refused by its permission check), and never say a page is open before it is.
